Health and Care Partnerships (HCPs)
HCPs bring together health, social care and voluntary sector organisations across smaller areas of our ICS to work as one. They play a fundamental role in the way health and care is provided for people in their areas, linking services to ensure people can get the care they need in the right place and at the right time.

This HCP covers Broxbourne, East Herts, North Herts, Stevenage and Welwyn Hatfield. -
South and West Hertfordshire HCP
This HCP covers Dacorum, Hertsmere, St Albans City and District, Three Rivers and Watford. -
West Essex HCP
This HCP covers Epping Forest, Harlow and Uttlesford. -
Hertfordshire Mental Health, Learning Disability and Autism Health and Care Partnership (MHLDA)
