This includes information held on computers, in emails and in printed or handwritten documents.
It also includes images, video and audio recordings.
And as an ICB, we are committed to being open and transparent and providing you with the information you need.
The Freedom of Information (FOI) Act 2000 allows you to make a request for information. Before doing this, please check whether this is already available elsewhere on our website.
The Act sets out how we should handle requests and outlines the exemptions to providing requested information. It also explains the ICB has 20 working days in which to respond.
Day one is the next working day from when the request is received.
We will decide if the FOI Act applies to your request. If we think your enquiry falls outside our duty under the Act, we will let you know and tell you why this is.
You can access any recorded information you think we may hold.
If your request is about the environment, we will reply to your request under the Environmental information Regulations 2004 (EIR).
If your request is about personal data that the ICB holds about you, you need to make a subject access request under the Data Protection Act 1998 (DPA).
Please complete this form for FOI or EIR requests, or use the details below.
